You can not clean that and the mold smell is in the pad. Either replace the pad or just leave it out. I never put pad under carpet. Now clean the carpet and add some mildew killer to the cleaning agent. They sell this at Lowes and Home Depot. Reattach the carpet at the wall with a carpet tacker prior to cleaning as it might shrink otherwise.
