If she is still unable to fix the problem, hopefully, she will remember that you were supposed to be added to the account. If she cannot get you on the checking account, then ask her to get her manager. As for the card, that is a separate account from the checking. It's not really a big deal for you to get your own, assuming your name is on the checking account. You should be able to take care of this on the same trip.
